The service also assists with planning for the future, offering emotional and practical support often in difficult circumstances. Also provide information on NHS services and assist patients to access health care, helps to solve problems offering practical support with form completion, housing application, and bus pass application, registration with GP, dentist or schools. They assist young adults to apply for college, offering support and encouragement with integration and activities.
